Background The State of Montana, through the State Procurement Services Division (SPSD) and on behalf of the Department of Public Health and Human Services, is issuing this Request for Proposal (RFP) to provide Wellness Kiosk Supplies and Services. The goal of this contract is to enhance public health initiatives by placing naloxone and fentanyl test strip vending machines across the state, addressing critical health concerns related to drug use. Work Details The contractor will be responsible for providing the following products and services: 1. **Products:** - Naloxone: 1 unit (EA - Each), cost per individual unit to be provided. - Fentanyl Test Strips: 1 unit (EA - Each), cost per strip to be provided. 2. **Services:** - Installation/Setup: Cost for shipping and installing each machine (1 LS - Lump Sum). - Monthly Lease: Cost for leasing each machine on a monthly basis (1 LS - Lump Sum). - Repair/Restock: Monthly cost for servicing and restocking each machine (1 LS - Lump Sum). Period of Performance The contract period will commence upon award and will continue until the completion of all services, with specific timelines outlined in the RFP documentation. Place of Performance The services and products will be delivered across various locations throughout the state of Montana. Bidder Requirements Offerors must comply with several requirements including registration with SAM.GOV to obtain a Unique Entity ID (UEI #), completion of Federal Form NDA889, adherence to insurance requirements, and acceptance of various prerequisites outlined in the RFP such as reviewing mandatory documents and confirming compliance with Equal Pay for Montana Women.
