STATEMENT OF WORK Project Title

Concrete Repairs at bldgs. 2, 45, 46 and 162. Replace multipurpose / basketball court at bldg. 160 at JHQVAMC Objective This project proposes repairing and/or replacing concrete deficiencies at various James H. Quillen VA Medical Center buildings, Mountain Home, Tennessee. Repairs include bldgs. 2, 45, 46,162 and 160 where the existing basketball court will be demolished and replaced with 6 steel reinforced concrete. The deficiencies pose a dangerous trip/fall hazards at various highly travel areas by patients, pedestrians and employees on station. Scope of Work Procedure B162: Infill concrete leveling foam, patch, remove trip hazards B45 and 46: Demo damaged sidewalk and install new 3500 psi re-enforced concrete B160 Courtyard: Demo existing BB court and replace slab with 6 steel reinforced concrete. Add stripping for foul and 3-point lines. B2: Replace damaged sidewalk on south side of Dogwood Ave approx. 120 sq ft Removal and Disposal: All demo debris to be removed from the property for proper disposal Procurement and Delivery: Contractor is responsible for all labor and materials.
